Clinical Assessment Specialist


This is a Full-time position in Baton Rouge, LA posted January 15, 2022.


Who we need:

Your comprehensive assessments will determine whether applicants and participants meet Level of Care and program eligibility requirements for participation in the Long-Term Personal Care Services (LTPCS) program.

If the assessment is approved, you will work to develop a Plan of Care to support the individual to stay in their home and community and routinely monitor their services.

You must have solid assessment/clinical skills and be able to administer the interRAI HC assessment and approved criteria and protocols.

This is a remote position, where you will have some flexibility in setting your own schedule.

Travel within a Louisiana catchment is required; so, LA residency is preferred.

We will accept full-time, part-time and PRN staff.

What you’ll do:

* Travel to and conduct face-to-face assessments visits for LTPCS applicants and participants (initial, annual reevaluation, and change-of-status assessments, per need)
* Accurately determine whether the individual meets Level of Care and program eligibility requirements for participation in the LTPCS program utilizing the interRAI HC assessment and approved criteria and protocols (includes review of medical documentation)
* Provide an opportunity for voter registration and assist with document completion when requested
* Develop/update/revise the eligible participants person-centered Plan of Care based on certification/recertification findings; prior authorize services approved on the Plan of Care
* Provide the participant with a freedom of choice listing of available service provider agencies and explain the freedom of choice process
* Confirm service delivery through telephone follow-up calls
* Participate in appeals if/when an applicant/participant is determined to not meet Level of Care and requests a fair hearing
* Case Maintenance/Quality Management: Perform monthly telephone contacts with participants and quarterly on-site visits to monitor services and ascertain that LTPCS service are being provided per the Plan of Care
* Utilize the states Electronic Visit Verification System to document all face-to-face visits
* Utilize Kepros proprietary care management system to document other tasks and activities
* Maintain medical records confidentiality through proper use of computer passwords, maintenance of secured files, adherence to HIPAA polices
* Utilize proper telephone etiquette and judicious use of other verbal and written communications, following Kepro policies, procedures, and guidelines
* Actively cross-train to perform duties of other positions within the KEPRO network to provide a flexible workforce to meet client/consumer needs.

What youll need:

* Bachelors degree in a Human Services field -or
– be a Registered Nurse (RN) with a current Louisiana license required
* At least one (1) year of experience working with older adults or persons with disabilities required
* Experience with the LTPCS program preferred
* Experience administering the interRAI HC assessment preferred.
